Each Additional Family Member after 3rd: $150/yr.

• Limited/Emergency Evaluation: 100% off (4 per year)
• Periodontal Consults: not covered by plan
Diagnostice Imaging:
• All X-Rays*: 100% off
• 2D Oral Photos: 100% off

Preventative Care:
• Adult or Child Cleaning: 100% off (2 per year)
• Fluoride**: 100% off (2 per year)
• Sealants: 50% off
Treatment:
